Allina Health System has been chosen by the federal Centers for Medicare and Medicaid Services to participate in a five year Accountable Health Communities (AHC) model test to assess if bridging the gap between clinical and community service providers for Medicare and Medicaid patients improves quality of health care and health care costs. Allina Health System was responsible for over 9% of 32,872 Medicare inpatient discharges for cardiovascular procedures in Minnesota - totaling 2,987 discharges within the year.
Mayo Clinic Hospital, located in Rochester, Minnesota, is the second highest volume hospital in cardiovascular Medicare inpatient discharges in Minnesota. Mayo Clinic accounts for over 8% of all Minnesota Medicare inpatient discharges involving treatments for cardiovascular conditions. Mayo Clinic surgeons are among some of the most innovatively equipped surgeons in the nation; they work with cutting edge technology to perform procedures such as minimally invasive, robot-assisted heart surgery. The cardiovascular team provides comprehensive care to their patients by collaborating with other hospital division physicians relevant to a particular patient’s case.
St. Cloud Hospital serves patients primarily in Central Minnesota. It was founded in 1886 as a small community hospital and has now grown into the largest health care facilities in its region. St. Cloud Hospital works with innovative technology and state-of-the-art-care for heart disease, as well as other specialty fields. In 2015, the facility was honored with the Quality Achievement Award for Heart Failure Care by the American Heart Association. St. Cloud Hospital had the third highest volume of Medicare inpatient discharges for cardiovascular procedures in the state. They totaled just over 2,500 inpatient discharges - nearly 8% of all discharges in Minnesota.
