Dexur’s analysis of medicare claims of hospitals in New York City showed that Hospital for Special Surgery, Lenox Hill Hospital, & NewYork Presbyterian - Columbia University Medical Cente had the lowest cost of care over a 90-day episode for Hip & Knee Replacement. The episode of care tracks a patient's journey from the date of admission for 90 days including the index hospitalizations, rehospitalization, home health, nursing home, and ED costs. The total cost of care helps patients make appropriate choices and helps local ACOs such as New York Quality Care, Family Health ACO, Community Care of Brooklyn, Richmond Quality, The Physicians Alliance, Alliance for Integrated Care of New York, and HHC ACO to make appropriate referral choices. The analysis was risk-adjusted and was done at the DRG level (specifically DRG 470: Major Joint Replacement Or Reattachment Of Lower Extremity Without Major Complication Or Comorbidity (MCC) to adjust for complications and comorbidities.

The study included hospitals within a 10 mile radius of New York City and with more than 100 medicare hospitalizations in a 3 year period (July 2017 to Jun 2020). A total of 16 hospitals made these criteria.

Hospital for Special Surgery’s total cost of care over a 90-day episode was $18,470 (for the period of Jul 2017 to Jun 2020). The Index hospitalization cost was $13,211 and the average rehospitalization cost was $1,463. The unplanned readmission rate (i.e. readmissions via the ED) was 2.75% and the cost per rehospitalization was at an average of $19,418.
