Based on a recent analysis performed by Dexur, two Baystate Health facilities were among the top five facilities in Massachusetts for lowest average length of stay (LOS) for a 30-day episode of care with unplanned readmissions. Baystate Mary Lane Hospital in Ware held the top spot with the lowest average LOS with unplanned readmissions at 3.45 days, while Baystate Franklin Medical Center in Greenfield averaged a 4.1 day LOS when including all COPD readmissions, the lowest in that category. Baystate Mary Lane Hospital also ranked 4th with all readmissions with an average LOS of 4.58 days. Baystate Franklin Medical Center ranked 3rd at 4.08 days for unplanned COPD readmissions. Dorchester’s Carney Hospital ranked 2nd in 30-day episode of care average LOS with unplanned readmissions at 3.8 days and with all readmissions at 4.36 days. The top five hospitals by average LOS for a 30-day episode of care including unplanned readmissions are shown in the graph below.

Massachusetts hospitals were incorporated into this analysis if they accounted for at least 50 DRG-190 (Chronic Obstructive Pulmonary Disease with Major Complication or Comorbidity) discharges between January 2013 and December 2016. DRG-191 (Chronic Obstructive Pulmonary Disease with Complication or Comorbidity) and DRG-192 (Chronic Obstructive Pulmonary Disease without Complication or Comorbidity/Major Complication or Comorbidity) are also included within the COPD condition. DRG-190 was the highest volume DRG of the three, making it best for this analysis. In the nation there were 594,304 DRG-190 discharges, compared to 471,758 for DRG-191 and 295,395 discharges for DRG-192. In Massachusetts there were 16,503 cases of DRG-190 after exclusion, which was the highest volume DRG after DRG-191, which had 13,535 discharges and DRG-192’s 7,383 discharges. All discharge totals excluded patients expired, were transferred, or discharged against medical advice.

Readmission rates are also relevant to the analysis of average LOS as it is critical that hospitals are not decreasing LOS at the expense of readmissions due to further complications in patients. Among the top hospitals were Carney Hospital ranked 9th in the state for readmission rates, readmitting 22.5% of all COPD patients, and Baystate Franklin Medical Center had a readmission rate of 21.43%, which ranked 5th overall in Massachusetts readmission rates. As the smallest hospital in this analysis, Athol Hospital had less than 11 readmissions for DRG-190 however based on their total DRG-190 discharge volume of 57, it can be calculated that the readmission rate is less than 17.54% (the readmission rate assuming there were 10 readmissions).
