Frailty is a condition marked by signs of declining vitality and health outcomes such as fatigue, weakness, poor physical activity, slow walking speed, and weight loss. According to several studies, frailty is common among older adults and approximately 10% of individuals, 65 years or older, are frail. Frailty is associated with an increase in rehospitalization, in-hospital mortality, longer length of stay, and higher hospitalization costs. This can adversely impact quality programs such as Value-Based Purchasing (VBP), Hospital Readmissions Reduction Program (HRRP), Bundled Payments for Care Improvement - Advanced (BPCI-A), and CMS Star Rating.

Dexur’s analysis of medicare claims of hospitals in Southwestern Florida showed that University of Miami Hospital is at Southwestern Florida’s Benchmark for Total Cost of Care for Heart Failure. The analysis was done for the period between April 2016 and March 2019. The episode of care tracks a patient journey from the date of admission for 90 days includes the index hospitalizations, rehospitalization, home health, nursing home, and ED costs. The total cost of care helps patients make appropriate choices and helps local ACOs such as ACMG Health Systems, Orange Accountable Care, Organization of South Florida, and Genuine Health ACO to make appropriate referrals choices. The analysis was risk-adjusted and was done at the DRG level, specifically DRG 291: Heart Failure And Shock With Major Complication Or Comorbidity. Read more
