Dexur’s analysis of Medicare claims data from Oct 2016 to Sep 2019 shows that Hill Top Home of Comfort is among the top ranked nursing homes in North Dakota with the lowest hospital readmission rates. CMS has initiated new programs to measure readmissions when a patient is discharged from a hospital to a skilled nursing facility (SNF) and then readmitted back to any hospital within 30 days of the original discharge.
