Dexur analyzed Medicare claims data from Oct 2015 to Sep 2018 to understand post hospital readmissions via the nursing home. Patients were tracked from the date of hospital discharge back to a skilled nursing facility (SNF) and back again to a hospital within 30 days. This information helps patients and hospital quality outcomes professionals be better informed and improve quality outcomes.
