A June 4 Miami Herald investigation found that according to CMS data, Jackson Health System performed under the national benchmark on incidence of MRSA bloodstream infections, but at or above the national benchmark on five other types of infection. Dr. Lilian Abbo of Jackson Health attributed this to the case mix at Jackson Health and the high prevalence of MRSA in Miami-Dade County.
To examine whether these MRSA findings are transferable to other infections that start cutaneously, Dexur analysts examined the incidence of acute bacterial skin and skin structure infection (ABSSSI) in 2013-2016 CMS inpatient discharge data from the 26 hospitals in Miami-Dade county that had over 500 Medicare-eligible inpatient discharges per year.
At these 26 hospitals, the average incidence of ABSSSI was 7.36%.
All three of Jackson Health's hospitals had somewhat lower ABSSSI incidence than average. 5.07% of discharges involved ABSSSI at Jackson Memorial Hospital; 6.10% at Jackson South Community Hospital; and 6.51% at Jackson North Medical Center.
To examine case mix (i.e. which hospitals have sicker patients), analysts examined ABSSSI in cellulitis specifically (DRGs 602 and 603) at 21 of these hospitals. At all hospitals, all or nearly all of these infections were present on admission, not nosocomial. 27.59% of Jackson North Medical Center's patients had major complications or comorbidities (MCC). This was second only to North Shore Medical Center, where 29.17% of cellulitis patients with ABSSSI had MCC. The lowest rate of MCC was at Larkin Community Hospital Palm Springs, where only 8.94% of cellulitis patients with ABSSSI had MCC.
