Dr. Shazia Abbas Discharged the Most Sepsis Patients in Iowa with Over 250 Discharges between 2013 and 2016

A recent analysis by Dexur of CMS Medicare claims discovered that Dr. Shazia Abbas of Central Iowa Healthcare* outperformed all Iowa physicians in terms of sepsis discharge volume between January 2013 and December 2016 with 257 total discharges. Dr. Pankaj Agrawal's 187 discharges ranked 2nd, which was 70 discharges less than Dr. Abbas. Dr. Agrawal was also associated with Central Iowa Healthcare*. Dr. Laurence M. Deshields followed closely in 3rd with 181 discharges. In 4th was Dr. Pinalkumar Patel with 166 discharges and Dr. Burt Humburg rounded out the top five with 149 discharges. The top four physicians for all sepsis Medicare discharges were also the top four discharging physicians for DRG-871 with Dr. Abbas also leading this ranking with 197 discharges. Dr. Deshields followed in 2nd with 158 discharges, while Dr. Agrawal had 132 DRG-871 discharges placing him 3rd. Dr. Patel kept his ranking at 4th with 116 DRG-871 discharges during the same time period.

Physicians were included in this study if they accounted for at least 11 discharges classified as sepsis or DRG-871 for each respective analysis. DRG-871 is one of three DRGs included within sepsis; DRG-871 (Septicemia or Severe Sepsis without MV > 96 hours with Major Complication or Comorbidity) is the most common sepsis diagnosis. DRG-872 (Septicemia or Severe Sepsis without MV > 96 hours with Major Complication or Comorbidity) and DRG-870 (Septicemia or Severe Sepsis with MV > 96 hours) are also sepsis diagnoses, although DRG-870 had fewer than 1,000 discharges in the state over four years.

Another point of analysis that may shed more light on each physician's quality of care is their average length of stay. LOS should be analyzed at the DRG level as the number of days admitted in the hospital varies dramatically with different degrees of severity in each DRG. Dr. Abbas had the 6th lowest LOS for DRG-871 at 4.3 days. Dr. Deshields ranked 17th for DRG-871 with an average LOS of 5.2 days. Dr. Agrawal held the lowest average LOS of the top 4 discharging physicians, ranking 5th overall at 4.2 days. Ranking 14th overall for lowest average LOS for DRG-871 patients was Dr. Patel at 5 days. For comparison, Dr. Humburg had the lowest average LOS for DRG-871 at 3.8 days. He was 12th for DRG-871 discharge volume and 5th for all sepsis Medicare inpatient discharge volume.

*During the time period of this study, the facility was known as Central Iowa Healthcare. Since then it has been incorporated into UnityPoint Health as the Marshalltown campus.
