A recent Dexur analysis of inpatient Medicare claims data shows that Rockledge Regional Medical Center in Rockledge, Florida accounted for a 19.44% skilled nursing facility (SNF) discharge rate for sepsis, the lowest of all major hospitals in Central Florida throughout 2016. The hospital with the next lowest SNF discharge rate for the condition was Oak Hill Hospital in Brooksville, where the 21.32% rate was nearly 2% higher than the rate at Rockledge Regional. The 3rd ranked hospital was Medical Center of Trinity, located in Trinity, with a SNF discharge rate of 21.43%. The 4th ranked hospital was Tampa General Hospital in Tampa at 22.44% and the 5th ranked hospital was The Villages Regional Hospital in The Villages, which accounted for a 22.87% rate. These five facilities were the only Central Florida hospitals to report SNF discharge rates under 23% for sepsis during the analyzed time period.
Hospitals were included in the rankings if they accounted for at least 3,500 total Medicare inpatient discharges and a minimum of 11 DRG-871 (Septicemia or Severe Sepsis Without MV >96 Hours With Major Complication or Comorbidity) discharges between January and December 2016. 83 Florida hospitals met this criteria, including 27 within Central Florida. DRG-871 was selected for this analysis due to its high discharge volume within sepsis; 570,388 discharges were reported nationally at the DRG-level, including 33,105 from Florida Medicare enrollees. Comparatively, 163,827 discharges were reported at the national level for DRG-872 (Septicemia or Severe Sepsis Without MV >96 Hours Without Major Complication or Comorbidity), including 9,572 for Florida Medicare enrollees while only 34,149 DRG-870 (Septicemia or Severe Sepsis With MV >96 Hours) discharges were accounted for nationally, including 2,419 from Florida.
Interestingly, even with the hospital’s low SNF discharge rate in comparison to other regional facilities, Rockledge Regional Medical Center saw the least amount of sepsis discharges out of all major Central Florida hospitals during 2016 with 197; this was ten fewer than the 207 discharges accounted for by the 26th ranked Central Florida Regional Hospital in Sanford. It is important to note that SNF discharge rates are not necessarily an indicator of quality of care as SNF discharge rates are often reflective of the demographics within a patient population. In fact, another Dexur article explored the relationship between SNF discharge rates and average hospital length of stay for the 20 highest volume Medicare DRGs in 2016 and did not find a significant correlation between these two quality metrics for sepsis discharges.
