Dexur analysts performed a correlational analysis on Michigan hospitals treating Medicare inpatients with COPD and found a moderate positive relationship between average index hospitalization LOS and readmission rate. This correlation can be evidenced by hospitals that have both low average index length of stay and low readmission rates, such as Oaklawn Hospital and St. Joseph Mercy Livingston Hospital. Oaklawn Hospital in Marshall had the state's lowest index LOS at just 2.81 days as well as the state's 2nd lowest readmission rate, seeing only 13.97% of COPD Medicare discharges had readmissions within 30 days. St. Joseph Mercy Livingston Hospital led the state with the lowest COPD readmission rate at just 11.4% and had the 5th lowest average LOS for the index hospital stay at 3.29 days.

There were 70 total hospitals analyzed, all of which had at least 50 DRG-190 (Chronic Pulmonary Obstructive Disease with Major Complication or Comorbidity) discharges and at least 11 readmissions between January 2013 and December 2016. COPD also encompasses DRG-191 (COPD with Complication or Comorbidity) and DRG-192 (COPD without Complication or Comorbidity/Major Complication or Comorbidity), where DRG-191 is the highest volume diagnosis-related group within COPD. 45% of all COPD discharges in the state and 44% throughout the nation were classified as DRG-190. Average length of stay for the index stay only includes total days in the hospital for Medicare inpatients diagnosed with DRG-190 on their initial stay. The readmission rate includes the percentage of DRG-190 patients who were readmitted to any hospital, not only the index hospital, within 30 days.

This analysis yielded a correlation coefficient of 0.55, which suggests a moderate positive relationship between average index LOS and 30-day readmission rate. This correlation indicates that a lower average index LOS for DRG-190 is associated with a lower readmission rate for these hospitals. When both these metrics are low, it suggests that the hospital is able to efficiently treat patients in a short amount of time without increasing their risk of complications that would require further medical care after discharge. Both Oaklawn Hospital and St. Joseph Mercy Livingston Hospital were in the top five for a number of other quality metrics as well. Oaklawn Hospital ranked first for average LOS for a 30-day episode of care including unplanned readmissions (including days hospitalized during index stay and unplanned readmitted stay within 30 days) at 3.12 days while St. Joseph Mercy Livingston Hospital ranked 5th at 3.8 days. Furthermore, Oaklawn Hospital ranked 2nd for average 30-day episode of care LOS with all readmissions (including days hospitalized during index stay and any 30-day readmitted stay) at 3.56 days. St. Joseph Mercy Livingston Hospital averaged 3.84 days to rank 4th.
