Dexur analyzed Medicare claims data for the study period of Oct 2015 to Sep 2018, to find out the top hospitals that provide the most stays for Meridian Nursing and Rehab Subacute Rehabilitation, Skilled Nursing Facility (SNF) of Wall, New Jersey. The top three hospitals - Jersey Shore University Medical Center , Ocean Medical Center and Riverview Medical Center provide more than 70% of all stays to Meridian Nursing home (see below table)

Jersey Shore University Medical Center topped the list with a share above 50% of all SNF transfers to Meridian Rehab Nursing home. However, when we analyzed Jersey Shore’s SNF transfer rate, we noticed that Meridian at Wall, NJ only got 31% of total SNF transfers from that Hospital. Jersey Shore’s other top SNF partner was also the Meridian Rehab facility at Ocean Grove, NJ.
Some of the other top hospitals that are transferring patients to Meridian Subacute Rehabilitation SNF are Ocean Medical Center, Brick with 17.3% rate, Riverview Medical Center, Red Bank with a rate of 2.9%, Monmouth Medical Center, Long Branch, and CentraState Medical Center, Freehold, both with a rate of 1.8%.

Major Hip and Knee Joint Replacement or Reattachment of Lower Extremity is the top DRG driving SNF transfers with a share of 23% followed by Heart Failure and Shock with 5%, Hip and Femur Procedures Except Major Joint, Septicemia or Severe Sepsis with 4% and Renal Failure with 2% transfer rate.
