A recent study by Dexur analysts indicates that Dr. Aileen Garcia-Gayoso of Methodist University Hospital in Memphis accounted for 256 Medicare inpatient discharges with renal failure, the most of any Tennessee physician between 2013-2016. She was the only physician in the state with more than 200 discharges for this condition. Methodist University Hospital was also the practicing location for the next four highest volume physicians for the condition. Dr. Hany M. Habashy ranked 2nd with 184 discharges, while Dr. Randy Villanueva ranked 3rd with 154 discharges. Dr. Oluwatoyin J. Agbaosi ranked 4th with 152 discharges and Dr. Charles L. Carter ranked 5th with 150 discharges. These were the only physicians in the state with 150 or more renal failure discharges over the four year period.

The study analyzed CMS inpatient claims data between January 2013 - December 2016. The top 50 highest volume attending physicians in Tennessee for renal failure patients were included in the rankings. The lowest volume physician in the analysis accounted for 65 discharges. Of the three diagnosis related groups within renal failure, DRG-683 (Renal Failure With Complication or Comorbidity) was the highest volume DRG, both nationally and in Tennessee. Among the 1,217,348 national discharges for all renal failure DRGs between 2013-2016, 617,923 (51%) discharges were classified as DRG-683.

Along with accounting for the highest total renal failure discharge volume, Dr. Garcia-Gayoso also had the highest discharge volume for DRG-683 with 86 discharges. While discharge volume may attest to a physician’s level of experience in treating a certain condition, measures such as average length of stay (LOS) and readmission rates help portray a more comprehensive picture of quality of care. Quality metrics, as opposed to discharge volume, must be analyzed at the DRG level, as different DRGs vary regarding their presence of complications and comorbidities and are therefore inherently associated with varying quality outcomes. For DRG-683, Dr. Garcia-Gayoso’s average LOS was 3.8 days between 2013-2016, which was below the state average of 4 days for Tennessee Medicare enrollees and even further below the 4.22 day national average. In 2016 alone, Dr. Garcia-Gayoso’s average LOS was even lower at 3.4 days. This was also substantially less than both state and national averages of 3.84 and 4.05 days, respectively.
